The Reel of Fortune takes one drip creature and gives back a skin. It sits on every island from the second onwards, and the machine on each island can only produce skins for the items that island’s machine lists.
How a spin is built
The machine fills ten reel slots. All ten start as a common skin. It then picks three of those ten at random and overwrites them: two become rare, one becomes legendary. Finally it draws the winning slot, uniformly, from the same ten.
That gives fixed rarity odds on every spin, on every island:

Nothing carries over between spins. There is no pity counter and no streak protection.
Which item you get
Once the rarity is settled, the machine picks an item from the pool that island’s machine stocks, and then a skin of that rarity belonging to that item. Both picks are uniform.
That means a smaller pool is better if you want something specific. The island 4 and island 5 machines each list three items plus the boat, four outcomes in all, so they are the most concentrated machines in the game; the island 2 machine spreads a spin over six.

The odds of one exact skin
Multiply the three draws together: the rarity slot, the item, and the skin within that rarity. A legendary on a machine with four possible outcomes, for an item that has three legendary skins, is 10% divided by four divided by three, which is 0.83% per spin.

Practical advice Spin on the island whose machine lists the fewest items, and prefer items with fewer skins at the rarity you want. Both shrink the denominator.
What a spin costs
One drip creature, or one quest item. Drip variants come off the normal catch roll at a flat 10% on any non-boss creature, so a spin costs about ten catches of fishing rather than money. The machine also accepts any item flagged as a quest item, which includes the five boss trophies in the boss drops lookup. That is a trap rather than an option: each trophy exists once and opens the next island or the ending, so hand it in and pay for spins with drip creatures.
Feeding a drip creature to the machine does not remove it from the pool. Catching another is a matter of casting again, which makes the machine’s real cost time rather than anything scarce.
